Before we start working together, you’ll need to complete our Client Information and Therapy Agreement forms. If we are going to be seeing your child and if both parents won’t be at the first appointment, be sure to have the non-attending parent sign the Therapy Agreement before the child comes in. 

If a non-custodial parent has had his or her parental rights revoked and/or has no contact with the child, the custodial parent can sign a statement to this effect, have it notarized, and bring it to the first appointment in lieu of the second parent’s signature on the Therapy Agreement.

If you would like us to communicate with another provider or someone else regarding our work with you, download and complete a Disclosure of Information form, either for Dr. Flemons or Dr. Green, and bring it with you to your appointment.
